Dr. Mo Alhezayen is a distinguished board-certified Internist with over two decades of experience in medicine. After completing an expedited seven-year MD program at The George Washington University, he graduated in 2002 as one of the youngest in his class. He further honed his expertise through an Internal Medicine residency at St. Joseph’s Hospital and Medical Center in Phoenix, Arizona, becoming board-certified at the remarkable age of 27. With a passion for patient-centered care, Dr. Alhezayen has built a diverse and dynamic medical career. He has practiced in multiple states and currently holds medical licenses in Texas, Arizona, and Oklahoma. His expertise spans inpatient and emergency medicine, along with advanced certifications in Advanced Cardiac Life Support, Pediatric Life Support, and Advanced Trauma Life Support. Dr. Alhezayen has expanded his focus to include cutting-edge regenerative therapies and biologics, providing innovative treatments designed to optimize health and healing. His extensive network of multidisciplinary specialists enables him to ensure his patients receive comprehensive, high-quality care tailored to their individual needs. Since 2011, Dr. Alhezayen has called the Dallas-Fort Worth area home. He is currently dedicated to providing medical consultations for surgical and trauma patients at Texas Health Presbyterian Hospital in Plano, while also serving as the Medical Director of ATA Healthcare and Wellness Clinic. Outside of his professional endeavors, Dr. Alhezayen enjoys spending quality time with his family and staying active. A lifelong sports enthusiast, he relishes playing soccer and cheering on his favorite local teams, including FC Dallas, the Mavericks, and the Cowboys. With over 20 years of leadership experience in clinical research, quality, and regulatory roles at major multinational corporations, he is recognized as an expert in advancing healthcare standards and practices. Dr. McCall completed his doctoral studies at the University of Southern California, focusing on improving patient care standards through risk mitigation and quality principles. He is an active member of several professional organizations, including the Collaborative Institutional Training Initiative, Society of Clinical Research Associates, Regulatory Affairs Professionals Society, American Society for Quality, Project Management Institute, and The Organization for Professionals in Regulatory Affairs. Through these affiliations, he has contributed to advancing industry best practices, standards, and regulatory frameworks. In his role as an institutional official, Dr. McCall oversees the Human Research Protection Program, ensuring the rights, safety, and welfare of human subjects are safeguarded. He also manages the Institutional Review Board (IRB), ensuring all research adheres to ethical standards and federal regulations. In addition to his professional endeavors, Dr. McCall has taught graduate-level courses in clinical research, quality, and regulatory science at various universities, sharing his extensive knowledge with future industry leaders. His global experience—spanning five deployments across five continents and establishing strategic operations in 24 countries—has shaped a mission-focused, results-driven approach that elevates every facet of our operations. Floyd’s commitment to helping others thrive aligns directly with NeoForm’s vision of restoring vitality through innovative, patient-centered care. His expertise in leadership, mental resilience, and strategic execution enables our team to deliver personalized, holistic healing experiences. Outside of NeoForm Wellness, Floyd is a respected national speaker, published author, and former city councilman. As a veteran of our country, he was deployed as a platoon leader in support of Operation Iraqi Freedom as a member of the Wisconsin National Guard in 2005-2006. He is a best selling author and keynote speaker. Nate has been in the Regenerative Medicine field since 2023.

Grant’s journey into wellness began with a lifelong passion for health and fitness, which he developed early on through sports. Starting athletics at a young age, he learned the importance of discipline, resilience, and prioritizing his physical well-being. This dedication ultimately led to a six-year professional basketball career, allowing him to travel the world and experience diverse cultures. His career brought him to Dallas, Texas, where he was drafted by the Dallas Mavericks NBA G League team. However, before he could take the court, severe knee injuries abruptly ended his time as an athlete. When faced with the option of major surgery, Grant sought a different path. Regenerative therapy not only healed his knees but also reshaped his understanding of health and recovery. This transformative experience revealed the limitations of traditional medicine and ignited a passion to help others discover innovative, non-invasive solutions for pain and injury. In addition to his work in wellness, Grant is a professional speaker who inspires audiences across the country. His talks focus on the power of belief as a foundational key to all success, sharing insights from his own journey to motivate others to overcome challenges and achieve their goals. Today, Grant dedicates his time to raising awareness about regenerative therapies and helping others take control of their health. Her proficiency in the financial operations of healthcare organizations, combined with experience guiding companies through strategic growth and scaling operations, strengthens multiple layers of NeoForm’s financial foundation.
Throughout her career, Puanani has worked with organizations focused on improving quality of life for patients. That thread continues at NeoForm, where her commitment to sustainable growth aligns directly with the company’s mission of expanding access to regenerative care.
Puanani is married to her high school sweetheart, Shiloh. Current residents of Southern Utah, they both grew up in American Samoa, and their culture and heritage remain an important part of their lives. Together they enjoy raising their children, staying active, tackling multiple projects, cooking and traveling together – especially to anywhere near the ocean! Faith and service remain at the center of their lives.

Dave has immersed in the Personal Development, Regenerative Therapy, and Health u0026 Wellness Industry for over 40 years. He is an authority and well recognized in this space. At the age of 19 he moved to the Dominican Republic where he spent two years as a volunteer missionary, sharing the good news of the Gospel of Jesus Christ, and he continues to serve as a volunteer minister in his church today.

Angela has trained and spoken to teams at organizations such as Delta Airlines, the CDC, Air Liquide, and the Tony Robbins organization, where she served on the leadership team supporting thousands of people in creating lasting change.
Her passion for holistic, functional, and regenerative wellness grew out of her own health journey and a deep conviction that true healing comes from addressing root causes rather than masking symptoms. Today, she is dedicated to educating and empowering individuals to explore natural, non-invasive approaches that support vitality, resilience, and long-term well-being.

I began my healthcare career at age 17 as a Certified Nursing Assistant in a skilled nursing facility. I subsequently gained over 15 years of nursing experience across diverse specialties, including medical-surgical
care, oncology, cardiovascular and orthopedic nursing, pediatrics and pediatric intensive care, labor and delivery, newborn nursery, neonatal intensive care, geriatrics, and postpartum care.
I also have a strong background in nursing education, with eight years of experience as an Assistant Nursing Professor during my RN years, followed by one year as an instructor in both the Nursing and Family Nurse
Practitioner programs at Weber State University.
Following my nursing career, I have practiced for 17 years as a Family Nurse Practitioner, providing comprehensive care to patients across the lifespan—from infancy through geriatrics. My clinical expertise
includes acute and chronic disease management, bioidentical hormone replacement therapy (bio-HRT), psychiatric care, weight management, women’s and men’s health, regenerative medicine (stem cell therapy and
peptides), and integrative care approaches to include functional medicine.
Clinically, I am skilled in a wide range of procedures, including skin biopsies, wound care, suturing, casting, podiatric procedures, IUD placement, Pap smears, and other general outpatient procedures. I possess specialized expertise in geriatric care, including dementia management, internal medicine, and psychiatric conditions.
My additional experience includes 15 years in hospice and palliative care, six years providing care in assisted living and skilled nursing facilities, seven years working in podiatry (including preventive foot care, injections, and ingrown toenail procedures), and three years as a qualified medical provider for medicinal cannabis. I also have five years of experience delivering care through house-call services for patients with limited mobility.
My passion for nursing began in childhood, inspired by my mother, who was also a nurse. From an early age, I felt a strong calling to care for others—a calling that has guided my professional journey from bedside nursing to advanced practice and education. Pursuing a post-master’s degree in family practice allowed me to expand my impact and deepen my ability to serve patients and their families. I am particularly passionate about community education and empowering patients and families through knowledge and compassionate care.
